Co Ed Basketball grades 1-4 (4th grade boys play co-ed). Girls Basketball grades 4-8& Boys Basketball grades 5-8 all start in October & the season ends mid March.

******* This cutoff date has been instituted because of the late number of signups in previous years. We need to know how many players are interested, so we can assign teams, get coaches, have enough uniforms ready and notify the different leagues of our intentions. If your permission form is received after the cutoff date, your child will be put on a waiting list and will only be added to a team is there is room.

Send in a signed Permission Form to the School Office (copies can be obtained on the school website or at the school office) – only choose 1 sport from the list above.
Also a Valid Sports Medical Physical Form must also be on file with the School Nurse. If your child’s medical form at the school is NOT valid you will be notified. The Medical Sports Physical Form is good for 1 year with one exception. If your Medical Form is valid the first day of that sport’s practice it will be considered valid for that sport’s season. The school office has Sports Medical Forms or it is also on the school web site-click on athletics.

By playing for a Sports Team at Queen of Heaven make sure you attend practices and be available for games. Sports schedules & seasons overlap-some school sports simply do not work out playing along with other school activities and outside interests.

** Both forms have to be turned into the School Office or given to the coach before any player can practice.
