Queen of Heaven Registration Forms and Requirements
2024-2025 School Year

Registration Fee:   $100.00 (non-refundable) Per Child .   
Re-Registration fee is $150 after March 31, 2024. 

Please make all checks payable to “Queen of Heaven School.”

Student Technology and Consumable Fee (STaC) $200 per family (non-refundable) due with registration fee.

A Complete New Student Registration Packet Includes: 
  • Registration Paperwork filled out completely (Email to [email protected]) 
  • Registration fee
  • HSG Family commitment form completed
  • ACH Form (if paying monthly)
  • Catholic Tuition Grant Program (CTGP) Form completed  (For parishioners of parishes other than QH)
  • Birth Certificate
  • Immunization Record
  • Transcript Request Form (For students transferring in from another school)
